Registration Policy
Purpose - The purpose of the policy is to ensure continuity and unity within the registration process. Registration of players will be consistent with District VIII and CYSA constitutions, bylaws, and policies and procedures. At the same time, MLYSL will regard collection and retention of personal information very seriously and will strictly observe all player and family privacy with respect to all information collected.
1. Player Registration Windows - The recognized seasons for MLYSL programs are:
Recreational: Regular = March 1 to May 7
Late = May 8 to June 30
Space Available = July 1 to September 1
Registration will begin on the 1st Saturday in March
7v7 Girls winter Tryouts = September 10 - October 15
All Registration packets to League Registrar
By October 15. NO EXCEPTIONS
Fill-ins = Upon approval of League Registrar
Bill Meyer: Tryouts = Beginning Date as set by District
Fill-ins = Any time after tryouts
Jim Frazier: Tryouts = Beginning Date as set by District
Fill-ins = Any time after tryouts
Kaercher (Division 3): Tryouts = Beginning Date as set by District
Fill-ins = Any time after tryouts
DOL (Division 1): Tryouts = Beginning Date as set by District
Fill-ins = Any time after tryouts
2. Bill Meyer; Jim Frazier; Kaercher and DOL Player Registration - players shall submit their registrations via the current MLYSL process. MLYSL shall establish a due date for teams to be submitted to allow time to comply with District requirements.
3. Recreational Program Player Registration - New and returning U6 through U15 recreational players shall submit their registrations via the current MLYSL process. 7v7 Girls Winter new and returning players shall submit their registrations via the current MLYSL Policy and within the guide lines listed under #1.
4. Registration Form - Every player playing in MLYSL shall have a completed CYSA Player Registration Form (Form 1601). The parent signature block must be completed for the registration to be processed.
5. Fundraising – Every player is required to participate in the MLYSL fundraising event if one is planned for that season. As a parent/guardian you have the option to pay an amount set by the MLYSL Board for the buyout (option not to participate). If you agree to participate and do not sell the required amount of items your child may not be able to participate with MLYSL in the future.
6. Team Area Definition - The team area of a player is based upon the physical home address listed on the registration form. MLYSL follows the ACUSD school boundaries. If asked or questioned about a player’s address, player’s family must provide utility bill for proof of address. Exceptions shall be made through the special request process established by MLYSL.
7. Special Requests for U8 and above Only requests completed on the MLYSL approved “Special Request Form” and initialed and signed by parents and submitted at time of registration will be considered. Please do not list any special requests on your CYSA Player form 1601, as they will not be granted.
Special request forms are required for requesting the following:
Ø Playing outside of Team Area
Ø Playing up one age group
Ø Playing for a specific coach
Ø Playing on a team with another specific player
8. Special Requests for Under 6 players - Only requests completed on the MLYSL approved “Buddy Draft Form” and initialed and signed by parents and submitted at time of registration will be considered. Please do not list any special requests on your CYSA Player form 1601, as they will not be granted.
Ø Player must qualify for the current U6 age division by their date of birth.
Ø Only one buddy per current player may be requested.
Ø Both players must request each other with this form.
Ø Request is subject to space availability on a team’s roster and is not a guarantee.
All special requests for all age groups will go into the “special request box” and these requests will be approved or denied by the MLYSL Board of Directors at the June meeting based on the needs of the Team area as of the meeting date. Special requests that are granted shall be for the current soccer season only.
Once a special request is granted, the player shall be registered to the appropriate team for the current season. A request to change the team assignment shall be processed as a transfer, and a transfer fee shall be collected.
9. Requests to Play Up - Upon request of a U12 player and in the 7th Grade, by the parents using the SPECIAL REQUEST FORM, and approval of the MLYSL Board of Directors, a player may play up one age group category. There will be NO MOVE UP Requests granted until the above requirements are met.
10. Requests to Play Down - Play down requests for medical reasons will be handled via consultation with the Area Coordinator and MLYSL Registrar. Medical Release may be required. There are no other play downs allowed.
11. Registration Fees - Registration fees are as follows:
· All registration fees will be set by the MLYSL Board of Directors on a year to year basis
· Discount for multiple players in same family: second player and beyond is regular registration fee less $10.00. Discounts apply to recreational fees only.
· All Reduced registration fees will be set by the MLYSL Board of Directors each year

12. Late Registration - Late registration will begin May 8 and will end September 1. Players registering after the registration deadline are placed in a Player Pool. Players must complete the entire registration process. The registration packet will not be processed until the player is placed on a team. Player Pool players are placed on a first come, first served basis per age division. Players may stay in the Player Pool until roughly two weeks after the season start date. In the event a player cannot be placed that season, all documents on the player will be shredded.
13. Refund Policy - MLYSL registration fees are refundable on the following schedule:
· Prior to July 1: Full refund less CYSA Player fees and insurance
· August 1 to August 15: 50% refund less CYSA Player fees and insurance
· After August 16: No refund
14. Transfer Policy - Recreational U15 and below - A player may move from one team to another team if there is space available by completing a transfer form (CYSA 1606). A transfer fee of $30.00 shall accompany the request. The request shall be forwarded to the MLYSL Board of Directors for approval or denial. Transfers shall only be allowed between August 1 and August 30. If the request is denied, the fee will be refunded. Bill Meyer, Jim Frazier, Kaercher and DOL - Transfers are to be entered on the transfer form (CYSA 1606) and processed per the District procedures. A fee may be charged by the District.
15. Rosters - Rosters will be frozen as of July 31 of each year. No transfers from team to team will be allowed without a written request (Form 1606) and permission from the MLYSL Board of Directors and payment of fee.
16. Coaching Registration - Coaches and assistant coaches must complete:
· CYSA Form 1628, Team Management/Adult Disclosure signed form for each team coached
· A 1.25”x1.50” picture. A good recent photo --no sunglasses, masks or hats that cover the face, not too dark or too light and it needs to look age appropriate.
Registration for all coaches is closed one week prior to season start. No coach or assistant coach will be added after this date. A coach’s pass will not be printed or distributed to a coach without a completed registration. There must be a pass-carrying coach or assistant coach present with the players at all times. It is the coach’s responsibility to complete the registration process and ensure they are in possession of their pass PRIOR to game day. Head coaches shall seek an assistant coach (es) (no more than two assistants registered to any recreational team) for the team. Assistant coaches must complete the same registration process as coaches. Having a properly registered assistant coach is to provide supervision to the players in the event of the coach’s absence.
17. Parent/Caretaker Ethics Agreement Form - Each player registration shall have a current year “Parent/Caretaker” form completed and signed. Registrations are not considered complete without this signed form.
18. Insurance Claims - When a player, coach, or official is injured while practicing, or playing, etc., a Youth Soccer Case Report shall be completed within 48 hours and forwarded to the MLYSL League Registrar. The Youth Soccer Case Report can be found in the back of the team manual that is issued to coaches every year. Alternatively the form may be downloaded at the www.cysanorth.org website. The MLYSL registrar shall process the form and forward it to State. In the team manual, there is also general information on the accident insurance program and how to file a claim once the primary insurance carrier has paid their share of the claim.
19. Requests for Scholarships - Reduced Registration fees - Only requests completed on the MLYSL approved “Reduced Registration Request Form”, initialed and signed by parents with minimum registration fee and submitted at time of registration will be considered. Proof of income is required by submitting a copy of your previous year’s 1040 Federal Income Tax Form (from the individual who claimed the scholar as a dependent). The Federal Income Tax form serves as proof of financial need as indicated upon the applicant's Scholarship Application. Scholarship money cannot be granted without receiving sufficient proof of income. Upon approval of the MLYSL Board of Directors, if the registrant is unable to pay the entire amount at one time, he/she may pay the amount in installments. The Scholarship request only covers the current registration fees. VOLUNTEER FEES ARE NOT WAIVED WITH SCHOLARSHIP. The MLYSL Board of Directors will require the parent or guardian of the registrant to provide four (4) hours of volunteer time.
20. Completed Registration Packet – will consist of:
o Completed and signed CYSA Form 1601
o Signed Parent/Caretaker Ethics Agreement Form
o Copy of registrant’s Certified Birth Certificate or an immigration visa, a passport or official government issued identification; (unless registrar has cleared player for having a certified copy already on file);
o A good recent 1.25 X 1.50 picture of registrant --no sunglasses, masks or hats that cover the face, not too dark or too light and it needs to look age appropriate.
o Full Payment
o Signed and completed Special Request Form if needed
o Signed and completed Buddy Draft Form if needed
o Signed and completed Scholarship Request form if needed
Area Coordinators, League Registrar or the League Treasurer: WILL Not accept any registration packet that is NOT COMPLETE.
Complete is defined as having the first 5 items and the appropriate Special Forms
INCORRECT OR INCOMPLETE REGISTRATIONS WILL BE RETURNED AND a $10.00 ADDITIONAL HANDLING CHARGE. Also received back to MLYSL after late registration is in effect, your registration will go into the Player Pool.
